Labiaplasty was first introduced into the scientific literature in 1971 by Capraro. Aesthetic labiaplasty was first published in the medical literature in 1984 by Hodgkinson and Hait. More recently, labiaplasty has increased in popularity due to increased awareness of labial corrective surgery and increased awareness of aesthetic ideals of female genitalia due to the growing popularity of “Brazilians” and more available explicit photography.

There remains significant questioning and criticism of labiaplasty by some in the media, medical community and society in general. It is sometimes argued that there is no indication for labiaplasty from a medical viewpoint and no need for such surgery from social circles who believe labiaplasty is for vanity and criticize the growing desire for “designer vaginas”.

There is evidence in the medical literature that shows that labiaplasty is a safe and effective procedure, and largely not performed for those seeking a designer vagina.

Many women who undergo labiaplasty do so for functional concerns such as pain, irritation, hygiene, dyspareunia (painful sexual intercourse), difficulty wearing tight pants, discomfort when cycling, horse riding and other physical activities. Other reasons for requesting labia reduction include embarrassment and self-consciousness of having labial hypertrophy. Some women are concerned by the stigma associated with having enlarged labia minora as a sign of having excessive sexual intercourse.

The causes of enlarged labia minora are probably multifactorial – congenital enlargement, androgenic hormonal excess, chronic infection (filarial) and chronic irritation and dermatitis.

A recent paper in the Annals of Plastic Surgery February 2012, surveyed 750 plastic surgeons to assess the trends in vaginal labiaplasty with regards to current techniques and their outcomes.

The survey showed that the commonest techniques employed by plastic surgeons included a straight line trim/amputation, central wedge excision, w-plasty, s-plasty and deepithelialisation. The techniques are broadly divided into those that resect the free labial margin and those that preserve the free labial edge.

Trim excisions, amputations, S-plasty and W-plasties resect the free labial edge and are often criticized for potential insensate scar along labial edge, scar contracture, inferior aesthetics at the labia-clitoral hood junction, advancing the posterior fourchette, chronic irritation and discomfort. Procedures that preserve the free labial margin such as central wedge excisions are often criticized for being complicated, narrowing the introitus, risking the deeper blood and nerve supply the labia, the steep colour change between the anterior and posterior labial edges and being more prone to wound breakdown.

Complication rate was low and consistent for all techniques of labial reduction. The commonest reason for reoperation was inadequate resection. Other causes for reoperation included wound dehiscence/poor wound healing, asymmetry, infection, haematoma, scar, fistula and stretching. The reported satisfaction rate by the patients was greater than 95%, which is comparable to other figures in the literature.

The results of this survey and other evidence in the literature would suggest that criticism of labiaplasty is largely unfounded. If performed for women who have symptomatic labial hypertrophy and with realistic expectations, vaginal labiaplasty can be a safe and effective procedure that improves women’s quality of life and can restore their confidence.

Liposuction is arguably one of the most popular cosmetic treatments in the world, and it is not only women who are requesting the surgery either. More men are having cosmetic surgery; some say it’s to keep up with the youthful appearance of their partners (one gentleman (53) had liposuction and an abdominoplasty (tummy tuck) so that he could keep up with his 33 year old girlfriend). As the popularity of cosmetic surgery increases with men so too does the age in which men are getting liposuction. Where once it was mostly men in their 30s getting liposuction now there’s a significant increase of men in their 5os and 60s getting it done.

So what is it?

Liposuction is a cosmetic procedure where small incisions are made into the targeted area so that tubes may be inserted. A vacuum is used to drain away fatty deposits from the target areas.

There are certain factors which determine a few things:

• Firstly, there is the decision between local or general anaesthetic (which you can discuss at the consultation).
• Secondly, the procedure time depends on the targeted areas.
• Small procedures can be done in a day.
• More complex ones may require a hospital stay overnight.

Recovery time is also dependent on how much you had done. A compression bandage is worn post-operatively for a couple of weeks and you are encouraged to do some light walking but no other exercise for four weeks.

Generally speaking recovery time for liposuction is up to a week but again it depends on just how much you have had done.
